Depression Self-Assessment Using PHQ-9

The PHQ-9 (Patient Health Questionnaire-9) is a widely used screening tool for depression. It evaluates how often you have experienced specific symptoms over the past two weeks. This self-assessment can help you understand your mood patterns but is not a medical diagnosis.

How to Use This Calculator

  1. Read each question carefully.
  2. Select the option that best describes your experience for each symptom.
  3. You can navigate between questions using the Next and Previous buttons.
  4. After completing all 9 questions, click Submit to see your results.
  5. Your results will include your score, severity level, and a breakdown of your symptoms.

Understanding Your Results

The PHQ-9 provides a score from 0 to 27. Your score corresponds to the following levels:

  • 0–4: Minimal or No Depression – Normal mood, little or no depressive symptoms.
  • 5–9: Mild Depression – Mild symptoms; lifestyle adjustments and monitoring may be enough.
  • 10–14: Moderate Depression – Moderate symptoms; consider talking to a counselor or professional.
  • 15–19: Moderately Severe Depression – Significant symptoms; professional support is recommended.
  • 20–27: Severe Depression – Severe symptoms; seek immediate professional help.

Symptom Breakdown

The PHQ-9 evaluates the following nine symptoms:

  1. Little interest or pleasure in doing things
  2. Feeling down, depressed, or hopeless
  3. Trouble falling/staying asleep, or sleeping too much
  4. Feeling tired or having little energy
  5. Poor appetite or overeating
  6. Feeling bad about yourself — or that you are a failure
  7. Trouble concentrating on things
  8. Moving/speaking so slowly or being fidgety/restless
  9. Thoughts that you would be better off dead or hurting yourself
